need some kind of support, guidance or clarification regarding a problem
you have been experiencing. We offer email, phone or office consultation
for individuals experiencing emotional or personal difficulties.
This is a first step in identifying how to begin resolving your problem.
We will then recommend whether you should consider some type of ongoing
counseling.
How
do I set up an appointment?
This
part is easy. Just complete the Short
Initial Counseling Form on CounselCare Connection, and
just press the submit button. It will automatically be sent to Lynette
Hoy's email. Or call Lynette at (708)
524-3333, ext. 1 to request a time. Payment can be made
by credit card or check as described below. Phone and office sessions
are 60 minutes.

Payment
can be made by check or cash or credit card at the time of
any phone or office appointment.
What
about office appointments?
If
you are living in the Chicagoland area you will find our ffice
location easily accessible. We are located at 1100 Lake Street,
Suite 245, just west of Chicago in downtown Oak Park, Illinois.
Just call 1-708-524-3333 for
an appointment. We schedule appointments for evening as well
as daytime hours Monday through Friday. Office sessions are 60
minutes
in length. Payment is requested at time of visit.
Is
there some kind of paperwork I need to complete?
Prior to a one hour *phone counseling session please submit email
or the Short
Initial Counseling Form You
will be sent some available
appointment times from which to choose. Call 708.524.3333 for an
office appointment if you prefer.
What
about confidentiality?
We
provide confidentiality for office appointments as long as
someone is not in danger of harming his/herself or someone
else or has not been abused. Though every effort is made to
provide complete confidentiality, phone
or
email
training
and
counseling
are not completely secure. But, we are committed to keeping
what you share during counseling
confidential
and will use reasonable precautions to maintain the privacy of
each session or consultation.
What
are the credentials of the Counselor?
Lynette
Hoy is credentialed by the National
Board for Certified Counselors as a National Certified
Counselor.
She was designated a Licensed Clinical Professional Counselor
in 1990 after working over 24 years as a Registered Nurse in the
State of Illinois. Lynette is a member
of the Christian Care Network with
the American Association of Christian Counselors and she is a
Rapha Network provider. Lynette
is a seminar
and conference speaker for professional and educational organizations,
church and community groups. Lynette is an American
Association of Christian Counselors Charter Member. She
is a
Blue Cross Blue Shield provider.
